[1] Take a new file of 400 pixels,400 pixels, of resolution 72 dpi in the RGB mode.

[2] Now take the Shape use the following settings.

[3] Create a shape fill with black color.

[4] Now go to Blending Option apply the following settings.

[5] Your image should be look like this.

[6] Now take the Text Tool write your text.

[7] Your image should be look like this.

[8] Now change the Blending Mode of Text Layer Overlay set the Opacity as shown image below.

[9] Your image should be look like this.

[10] Take the Shape Tool select the shape as shown below.

[11] Now create a shape.

[12] Go to Blending Optionn select the Gradient Overlay apply the following settings.

[13] Your image should be look like this.

[14] Now arange the Layers as show image below.

[15] Your final image should be look like this.
