[1] Lets take a new file of 400 pixels by 400 pixels then choose resolution 72 dpi in the RGB mode.

[2] Now take the Shape tool and select this one.

[3] Now create a new layer and draw a shape as shown image below.

[4] Then select the Direct Selection Tool and click the shape see in the next image.

[5] Now press the Cnlt+T to transform selection see in the next image.

[6] Your image should be look like this.

[7] Then go to Blending Option and select the Gradient Overlay use these settings.

[8] Do not click ok select the Stoke Structure apply the following settings.

[9] Do not click ok select the Drop Shadow apply the following settings.

[10] Your image should be look like this.

[11] Now create another shape with help of shape tool as shown image below.

[12] Then go to Blending Option and select the Bevel and Emboss use these settings.

[13] Do not click ok select the Gradient Overlay apply the following settings.

[14] Your image should be look like this.

[15] Now use the Text Tool & Shape Tool .

[16] Your final image should be look like this.
