[1] Take a new file of 300 pixels,300 pixels, of resolution 72 dpi in the RGB mode.

[2] Take the Shape Tool and select this one.

[3] Create a new shape and fill with Black color.

[4] Go to Blending Option use the following settings.

[5] Your image should be look like this.

[6] Take the Shape Tool and select this one.

[7] Create a shape and fill with White color.

[8] Go to Blending Option use the following settings.

[10] Take the Shape Tool and select this one.

[11] Create a shape and fill with White color.

[12] Now take the Elliptical Marquee Tool create a shape and fill with White color.

[13] Then change the Blending Mode and set the Opacity as shown image below.

[15] Finally create some text your final image is ready.
